caregiver talking to an elderly womanThe process in Universal Home Health Care, Inc. begins when you, your family member, or the physician’s representative contacts our agency. Our professional team of Registered Nurses and Care Team Coordinators will collaborate with physicians, discharge planners, social workers, and care managers to ensure that appropriate medical treatment is being provided to you or your loved one. The duration of care will be dependent upon your individualized needs. Our goal is to establish realistic objectives to assist in recovering your former functioning abilities. In order to meet these needs, a Nurse and/or Physical Therapist will be assigned to coordinate on your services.

This is the process that you can expect from our caring staff:

  • The Initial Assessment

    Our intake coordinators begin by reviewing your needs through discussions with the people involved – you, your family members, discharge coordinators, physician, social worker, etc.

  • The Needs Assessment

    Based on your physician’s diagnosis and treatment orders, our intake coordinators will discuss the type of care you will need. This assessment will allow us to determine the appropriate level of care for you – whether you need professional or paraprofessional services. Here, you will be able to know the duration of care you need – temporary, long-term, hours, days or evenings, weekends, etc.

  • The Initial Visit

    During the visit, we will explain about our available services, answer any questions you may have, and help with any important decisions you or your loved one encounters. Items addressed at this meeting include:

    • Matching the Appropriate Caregiver
    • Health and Medication Assessment
    • Nutritional Needs
    • Home Safety Inspection and Plan
    • Patient Bill of Rights and Responsibilities
    • Homecare Consent for Services
    • Patients’ Rights for Privacy
    • Client Concern Procedure
    • Advance Directives

    The Initial Visit or initial consultation will enable the nurse along with the physician to develop a care plan that will include:

    • Type of Caregiver
    • Physician’s orders
    • Caregiver responsibilities
    • Time and duration of care
    • Outline of care to be rendered
    • Medication Profile
  • Financial Discussion

    The Intake Nurse or Coordinator will discuss and document the financial responsibilities to you as well as the responsibilities of the agency.
